Cholesterol plays an essential role in the stability and architecture of the plasma membrane and as a precursor for bile acid and steroid hormone synthesis in mammals. Serum cholesterol is derived from the diet and is synthesized in the liver. Total cholesterol consists of free cholesterol and cholesterol esters. Cholesterol is a highly insoluble molecule that is transported in the circulation via endogenous transporters; the lipoproteins (HDL, LDL & VLDL).
Under healthy homeostatic conditions, the circulating levels of cholesterol are regulated by a balance between synthesis, dietary cholesterol and removal of the excess cholesterol from peripheral tissues. The disturbance of this homeostatic state is known to be associated with cardiovascular diseases and is often attributed to dietary and lifestyle habits.
Serum cholesterol level is affected by a number of factors, including genetics, diet, and tobacco usage, level of physical activity, age, and menopause. Quantification of total cholesterol allows the detection of hypercholesterolemia, isolated or associated with hypertriglyceridemia. High cholesterol concentrations are associated with a high risk for vascular accident and apparition of atherosclerosis. The LDL/HDL ratio are taken in consideration for evaluating the risk of developing cardiovascular diseases.
