Chronic inflammation is an important component in the development and progression of atherosclerosis, and numerous epidemiological studies have demonstrated that increased serum CRP concentrations are positively associated with a risk of future coronary events, such as coronary artery disease, cerebrovascular disease, or peripheral arterial disease. It has also been shown to be predictive of future events in patients with acute coronary syndromes and in patients with stable angina and coronary artery stents.

 

Coping with cardiovascular diseases (CVD), which are of the main causes of death worldwide, has influenced investigation of high sensitivity CRP (hsCRP) and its role in pathogenesis, prognosis and prevention of CVD. hsCRP can be synthesized in vascular endotheliumatherosclerotic plaques, and theory of inflammatory origin of atherosclerosis is being more widely debated, raising questions, whether higher hsCRP plasma concentration might be the cause or the consequence. Summing up controversial data from multiple studies, guidelines recommend hsCRP testing for both, primary (stratifying CVD risk groups, selecting patients for statin therapy) and secondary CVD prevention (prognosis of CVD and its treatment complications, evaluation of treatment efficacy in moderate CVD risk group). hsCRP testing also has role in heart failure, atrial fibrillation, arterial hypertension, valve pathology and prognosis of coronary stent thrombosis or restenosis.

 

Of the many markers, only hsCRP has fulfilled the required criteria for a novel marker of CHD risk, and national guidelines for its measurement in the primary prevention of CHD have recently been issued jointly by the American Heart Association and the CDC. The use of CRP for this purpose requires the use of hsCRP assays having detection limits less than 0.3mg /L.
